The issuance of the permit by the Department of Agriculture necessitates that the rabies vaccination issue date be at least 30 days prior. While you can initiate the application process before this period, the permit's release date will align with or exceed the 30-day requirement. The USDA APHIS form or a ā€œPet Passportā€ IS NOT required for entry. USDAā€™s APHIS website refers visitors back to the Bahamas Department of Agriculture form.


The Bahamas Pet Import Application has recently changed (July 2023) and many old versions are still available, such as this one from bahamas.com (official tourism website) Do not use the old permit form, download the correct application from my site.
